These Terms and Conditions constitute your agreement with the promoter in relation to your participation as a nominator in the promotion.

Health Professionals Bank, a Division of Teachers Mutual Bank Limited ABN 30 087 650 459, Australian Credit Licence No 238981, of 28-38 Powell Street, Homebush NSW 2140.
4. Eligibility to enter:
Australian resident adults 18 years and older are eligible to nominate a nurse or midwife working in Australia.
Self-nominations are not eligible.
Directors, employees and Members and Advisory Committee members of the promoter, and members of their family (spouse, de facto, parent, grandparent, child, stepchild, grandchild, brother or sister) are not eligible to enter or be nominated to the promotion.
5. Promotion period:
Starts: 9am AEST, Monday 5 May 2025
Ends: 5pm AEST, Friday 13 June 2025
6. How to enter:
To enter the promotion the nominator must complete the nomination form during the promotion period.
The nominator confirms that he/she has obtained the privacy consent of the nominee to be nominated in the nomination form.
All nominations must be received by the promoter before the end of the promotion period. Nominations are deemed to be made at the time of receipt by the promoter, not the time of transmission by the nominator. Nominations received after the promotion period will not be accepted.
To be considered a valid nomination, the nominee must be working as nurse or midwife in Australia.
The promoter will require the nominator to provide proof of identity, email and contact phone number. Proof considered suitable for verification is at the promoter’s sole discretion.
The nominator is not limited to one nomination for the Take Five Initiative.
By entering the promotion you accept these Terms and Conditions.
7. Judging details:
After the promotion period ends and on or before 27 June 2025, one valid nomination will be selected by a panel of industry experts and Health Professionals Bank employees. The selected nominator will be shared on the Health Professionals Bank website www.hpbank.com.au
The selection will be based on the most deserving nomination, as deemed by the judging panel.
The selection made by the judging panel is final.
This is a game of skill and chance plays no part in determining who is selected.
8. Prize:
The selected nominator will be selected to receive $1000 into a Health Professionals Bank or a Teachers Mutual Bank Limited Everyday Direct account.

9. Notification:
The selected nominator will be notified via phone call and/or email by 5pm AEST, Friday 11 July 2025.
10. Prize claim:
The prize winner must claim the prize within 5 days of being notified by phone or email by replying to the promoter’s notification email.
11. Announcement:
The winner details will be published on the Health Professionals Bank website on or before 5pm AEST, Thursday 31 July 2025.
12. Unclaimed prize:
Winners must claim their prize by 5pm AEST Friday, 18 July 2025. If any prize is not claimed by this time, the judging panel will select new winners for the unclaimed prizes on Tuesday, 22 July 2025, using the same criteria as applied for the original selection of the nominator.
13. Nominator’s consent for promotion:
By entering the promotion, each participant consents to the following if the participant wins the prize:
• their photo or video being taken by the official photographer or on own device;
• the promoter using their name, the nomination and photos and/or video of them in any media for an unlimited period of time for the purpose of promoting, publicising or marketing this promotion and promoting any products and services by the promoter;
• the promoter licensing others to use such photos for those purposes;
• the promoter being the exclusive owner in perpetuity of all rights in materials produced using the photos and all rights in those materials; and
• if photos or video are published, that third parties may view the photos without acknowledging the nominee.
14. Indemnity and disclaimer:
To the extent permitted by law, each entrant releases and indemnifies the promoter and its agents and employees from and against all liability, cost, loss or expense arising out of acceptance of any prize or participation in the competition including (but not limited to) loss of income, personal injury, damage to property and whether direct or consequential, foreseeable, due to some negligent act or omission or otherwise.
15. Limitation of liability:
To the extent permitted by law, the promoter is not responsible or liable:
• if for any reason beyond the promoter’s reasonable control, any element of any prize is not provided. The promoter, in its absolute discretion, may then substitute a different prize of equivalent value in place of any prize referred to in these terms and conditions;
• if, for any reason, the competition is suspended, varied or terminated at any time, with or without prior warning or publicity;
• for late, lost or misdirected entries; or
• for any change in the value of a prize occurring between the publishing date and the date the prize is claimed.
The promoter has the right to exclude any person from this competition for any reason whatsoever and to amend these terms and conditions at any time or cancel the competition subject to state regulation.
